Ischemic cardiomyopathy belongs to a special type or advanced type of coronary heart disease, which is a syndrome of long-term myocardial ischemia caused by coronary atherosclerosis, resulting in impaired myocardial contraction and/or diastolic function, causing a series of manifestations such as heart enlargement or stiffness, congestive heart failure, and arrhythmias. It is projected that the 1-year mortality rate of patients with ischemic cardiomyopathy is as high as 30%.
The basic cause of the disease is coronary artery stenosis or occlusive lesions caused by coronary artery dynamics and/or resistance factors.
The heart is different from other organs in the body in that its oxygen uptake rate in the basal state already accounts for approximately 75% of the coronary blood flow delivery, and when myocardial oxygen consumption increases, the demand for oxygen consumption can only be met by increasing the coronary blood flow.
There are several etiologies that can cause myocardial ischemia:
① Coronary artery atherosclerosis.
②Thrombosis.
③Vasculitis.
④ Other factors that can cause chronic myocardial ischemia include coronary microangiopathy (syndrome X) and structural abnormalities of the coronary arteries.
What should patients with ischemic cardiomyopathy pay attention to in general?
Eliminate all treatable or preventable risk factors of coronary heart disease, including smoking, hypertension, diabetes, hypercholesterolemia obesity control weight, etc. Pay attention to prevent the triggering factors of heart failure such as respiratory infections, too much and too fast infusion and other factors.
