I. Purpose
To strictly prevent errors in surgical patients, surgical sites, and surgical procedures, to ensure that the surgical safety verification system is effectively implemented, and to protect patient surgical safety.
II. Definition:
Surgical safety verification refers to the verification of patient identity, surgical site, surgical method, anesthesia and surgical risk, and inventory of items used in surgery by a tripartite group of qualified surgeons, anesthesiologists and operating room nurses before anesthesia is administered, before surgery begins and before the patient leaves the operating room, respectively. Verification of blood type and blood volume should also be included for patients who have received blood transfusions.
III. Verification records
Surgical safety verification should be a tripartite verification record, confirmed and signed by the surgeon, anesthesiologist and operating room nurse. The surgeon shall be the operator himself/herself, and under special circumstances (e.g., an outside specialist acting as the operator), the first assistant may be used instead.
Surgical safety verification content and process
(1) Before anesthesia implementation: the surgeon will preside over and be responsible for checking the contents of the verification before anesthesia implementation according to the Surgical Safety Verification Form, and all the contents of the verification before anesthesia start will be confirmed by the surgeon, anesthesiologist and the operating room nurse through the tripartite verification and signing of the full name in the corresponding position formally.

(b) Before the start of surgery: the anesthesiologist presides over and is responsible for checking the contents of the verification before the start of surgery according to the Surgical Safety Verification Form, and all the contents of the verification before the start of surgery are verified by the surgeon, the anesthesiologist, and the nurse in the operating room, and then the full name is formally signed in the corresponding position.
(C) Before the patient leaves the operating room: the nurse of the operating room hosts and is responsible for the verification, and all the contents of the verification before leaving the operating room are confirmed by the surgeon, anesthesiologist and the nurse of the operating room in a tripartite way, and then the full name is formally signed in the corresponding position.
V. Management of Surgical Safety Verification
(A) Surgical safety verification must be carried out sequentially in accordance with the content and workflow of surgical safety verification, and the next operation can only be carried out after each step of verification is correct, and the form must not be filled out in advance.
(b) Verification of intraoperative medication: the surgeon or anesthesiologist will give medical advice according to the needs of the situation, and the itinerant nurse will be responsible for verbally repeating it once to confirm that it is correct and then execute it, and leave the medication to the Ministry of Health for verification. The anesthesiologist is responsible for making appropriate records in a timely manner after the execution of the oral medical advice.
(c) After the three-party verification and signature, the "Surgical Safety Verification Form" is filed in the medical record.
(D) The director of each surgical department and anesthesiology department is the main person responsible for the implementation of the surgical safety verification system in his/her department.
(e) The Medical Department, Nursing Department and other medical quality management departments should, according to their respective responsibilities, conscientiously fulfill the supervision and management of the surgical safety verification work, put forward timely opinions and suggestions for continuous improvement and supervise the implementation.
The operating room is a high-risk medical place, the operating room work task is heavy, fast-paced robbery, any link of negligence may induce serious consequences. In the medical industry, there have been a number of accidents in which the surgical site or surgical patient was incorrect. If surgical error occurs, it will have a serious impact on both the patient and the medical staff, and it will also cause a huge economic loss and adverse social impact to the hospital, and the strict implementation of the safety verification system is a necessary measure to avoid these situations, so all medical staff should attach great importance to the surgical safety verification.
